Job summary

Mission Australia is seeking a Case Worker for the Homeless Youth Assistance Program in Broken Hill, NSW. You will work with young people aged 12 to 15, supporting family connections, education engagement and finding stable accommodation where needed.

Based in Broken Hill, this permanent role focuses on Aboriginal children and young people at risk of homelessness, with strong emphasis on partnership with government and non-government allies.

Qualifications

• Relevant qualification and/or experience in human services.
• Knowledge of homelessness issues including domestic violence, substance use, mental health.
• Computer literacy in a Windows environment; ability to use standard software.
• Current NSW Driver’s License and willingness to obtain First Aid if not held.

Responsibilities

• Respond to referrals and complete screening and intake with young people.
• Support accommodation needs of clients who enter the service.
• Develop individualised case plans and coordinate referrals to services.
• Provide ongoing case management to achieve identified goals.
• Build relationships with internal and external stakeholders and service partners.
• Complete a range of administrative duties for service operations.

Your Opportunity

Every young person deserves to feel safe, supported and connected. In the far west of New South Wales, some children and young people face homelessness, family breakdown and uncertainty at a critical stage of their lives.

As a Case Worker with Mission Australia’s Homeless Youth Assistance Program (HYAP), you will walk alongside young people aged 12 to 15, helping them reconnect with family, culture and community, or transition to safe and sustainable long-term accommodation where needed. Our Case Workers support young people to develop age-appropriate living skills, positively engage in education or training and to strengthen connections with family and wider support networks.

Based in Broken Hill, this permanent role provides a unique opportunity to make a lasting difference for vulnerable young people, with a strong focus on supporting Aboriginal children and young people who are homeless or at risk of homelessness across the Western NSW region.

Key responsibilities will include:

• Responding to referrals and completing screening and intake with young people.
• Supporting the accommodation needs of clients who enter the service.
• Working with clients to create individualised case plans and coordinating referrals to services.
• Providing ongoing case management to clients to achieve identified goals.
• Develop and maintain relationships with internal and external stakeholders & service partners, both government and non-government
• Complete a range of other administrative duties for the efficient running of the service.

Requirements for Success

• Relevant Qualification and / or relevant experience in the human services industry.
• Knowledge and experience working with client issues relating to homelessness including domestic violence, substance abuse, mental health etc.
• Computer literate in a windows environment.
• Demonstrated ability to work effectively as part of a team.
• First Aid Certificate or willingness to obtain.
• Current NSW Driver’s License.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
