Job summary

Mission Australia is hiring a Case Worker for its Homeless Youth Assistance Program (HYAP) in Dareton, NSW. You will work with young people aged 12 to 15 to reconnect with family, culture and community, and help transition to safe long-term housing where needed.

The role focuses on developing living skills, engagement in education or training, and strengthening support networks, with emphasis on Aboriginal children and youth at risk of homelessness across Western NSW.

Qualifications

• Relevant qualification and/or experience in the human services industry.
• Knowledge of homelessness issues including domestic violence, substance abuse, mental health.
• Computer literate in a Windows environment.
• Current NSW Driver’s License and ability to obtain clearance to work with children/vulnerable people.

Responsibilities

• Responding to referrals and completing screening and intake with young people.
• Supporting accommodation needs of clients who enter the service.
• Developing individualised case plans and coordinating referrals to services.
• Providing ongoing case management to achieve identified goals.
• Building relationships with internal and external stakeholders.

Your Opportunity

Every young person deserves to feel safe, supported and connected. In the far west of New South Wales, some children and young people face homelessness, family breakdown and uncertainty at a critical stage of their lives. As a Case Worker with Mission Australia’s Homeless Youth Assistance Program (HYAP), you will walk alongside young people aged 12 to 15, helping them reconnect with family, culture and community, or transition to safe and sustainable long-term accommodation where needed. Our Case Workers support young people to develop age-appropriate living skills, positively engage in education or training and to strengthen connections with family and wider support networks. Based in Dareton, this permanent role provides a unique opportunity to make a lasting difference for vulnerable young people, with a strong focus on supporting Aboriginal children and young people who are homeless or at risk of homelessness across the Western NSW region.

Key responsibilities

• Responding to referrals and completing screening and intake with young people.
• Supporting the accommodation needs of clients who enter the service.
• Working with clients to create individualised case plans and coordinating referrals to services.
• Providing ongoing case management to clients to achieve identified goals.
• Develop and maintain relationships with internal and external stakeholders & service partners, both government and non-government
• Complete a range of other administrative duties for the efficient running of the service.

Requirements for Success

• Relevant Qualification and / or relevant experience in the human services industry.
• Knowledge and experience working with client issues relating to homelessness including domestic violence, substance abuse, mental health etc.
• Computer literate in a windows environment.
• Demonstrated ability to work effectively as part of a team.
• First Aid Certificate or willingness to obtain.
• Current NSW Driver’s License.
• Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
