Job summary

Sacred Heart Mission’s Homefront program seeks a Case Manager to provide information, support and case management to women accessing crisis accommodation. The role is a 12-month full-time contract aligned with SHM’s values and funding requirements.

You will deliver person-centred support, assist with housing pathways, and work with partner agencies to ensure coordinated, compassionate care. A supportive environment and regular supervision are provided.

Qualifications

• Experience in case management or social services preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Provide person-centred support and case management to women residing in Homefront crisis accommodation.
• Deliver flexible case management and outreach support to women in crisis accommodation and housing pathways.
• Respond to day-to-day operations of the Homefront site with timely client support and a welcoming environment.
• Collaborate with colleagues and partner agencies to ensure coordinated client-centred support.
