Job summary

Home Caring is seeking a dedicated NDIS/Aged Care Case Manager to join our hybrid team, working from home, in the office, and in the community across the Mornington Peninsula region. The role requires experience managing both Disability and Aged Care clients with a person‑centred approach.

You will assess needs, develop care plans, coordinate services, and ensure quality standards are met, while liaising with families and health professionals in a culturally sensitive manner.

Qualifications

• Qualification in Nursing (Registered Nurse) and/or relevant Case Management experience.
• Experience in aged care, disability, or community health settings.
• Strong communication, problem-solving, and organisational skills.
• Understanding of NDIS and/or Aged Care systems.
• Empathy, cultural awareness, and a passion for helping others.
• Current AHPRA registration (for RNs.)
• Current First Aid & CPR, WWCC and NDIS Worker Screening
• Driver's Licence and access to reliable vehicle.
• Full working rights in Australia

The Case Manager will support a growing number of clients in the Mornington Peninsula and surrounding area. This role is a hybrid role and is a mix of WFH traveling to clients in the community across and in the office coordinating associated administration and engagement events.

This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You'll be working with clients across both Aged Care and Disability sectors.
