Job summary

St John Ambulance Victoria is seeking a Case Manager to join the Seniors Community Care team at our Somerville office, providing efficient and effective case management to help older people live safely and independently at home. The role is based in Somerville on the Mornington Peninsula.

• Registered Nurse or Enrolled Nurse qualifications are highly desirable but not essential.

About the role

We are seeking a Case Manager to join the Senior Community Care team to provide efficient and effective case management services to clients of Seniors Community Care Services. This role assists older people to remain living safely and independently in their own home and community by facilitating and coordinating the provision of care arrangements that are suitable for their individuals needs.

This role is based at our Somerville office.

As our ideal candidate, you will possess:

• Minimum of Diploma Qualification in Community Services, Social Work and/or Case Management
• Proven experience in case management, preferably in a community, aged care, or multicultural setting
