Job summary

Mission Australia is seeking a dedicated professional to provide psychosocial support services in Artarmon. You will deliver trauma-informed, recovery-focused case management to individuals with complex mental health and psychosocial needs, coordinating services to help clients achieve recovery and community connection.

The role requires a degree in Social Work, Psychology, or related field, with experience handling diverse caseloads and strong administrative skills, including data management

Qualifications

  • Degree in Social Work, Psychology, Community Services, Mental Health or related discipline.
  • Experience managing a caseload with complex mental health or homelessness needs.
  • Knowledge of recovery-oriented, trauma-informed approaches.
  • Strong communication and professional boundary setting skills.
  • Experience with case notes, reporting and data management systems like Excel.

Responsibilities

  • Deliver person-centred, trauma-informed case management to individuals with complex challenges and recovery goals.
  • Respond to referrals, complete intake assessments, determine eligibility, and develop SMART recovery plans.
  • Help participants access psychologists, GPs, housing, Centrelink and other supports.
  • Build relationships with participants, carers, external providers, and stakeholders to coordinate outcomes.
  • Monitor progress through wellbeing reviews and case plan updates; ensure engagement.
  • Maintain accurate case notes, referrals, and documentation per quality standards.

Your Opportunity

Make a meaningful difference in the lives of people experiencing severe and persistent mental health challenges through Mission Australia's Psychosocial Support Service in Artarmon. Working within a recovery focused framework, you will provide person centred, trauma informed case management that supports participants to achieve their goals, strengthen independence, and access the services they need to improve their wellbeing. This is an on-site position where you will manage a diverse caseload, coordinate supports across health and community services and help participants build sustainable pathways towards recovery and community connection. This rewarding role combines hands on participant support with care coordination, advocacy, and stakeholder engagement. You will conduct assessments, develop goal focused support plans, connect participants with appropriate services, and monitor progress, ensuring individuals remain engaged and supported throughout their recovery journey.

Your Key Responsibilities
  • Deliver person centred, trauma informed case management to individuals experiencing complex mental health and psychosocial challenges, supporting them to achieve recovery focused goals.
  • Respond to referrals, complete intake assessments, determine eligibility for support, and develop tailored recovery and case management plans using SMART goals.
  • Support participants to access and engage with psychologists, GPs, alcohol and other drug services, housing providers, Centrelink, and other community based supports.
  • Build and maintain positive relationships with participants, carers, families, external service providers, and community stakeholders to achieve coordinated outcomes.
  • Monitor participant progress through regular wellbeing reviews, case plan reviews, and ongoing engagement strategies that encourage motivation and sustained participation.
  • Maintain accurate case notes, referral documentation, reports, data entry, and client records in line with organisational requirements, quality standards, and compliance frameworks.
  • Contribute to service improvement initiatives, participate in internal and external network meetings, share knowledge of local services, and support collaborative team outcomes.
Requirements for Success
  • Degree in Social Work, Psychology, Community Services, Mental Health, or a related discipline, or equivalent experience within the community services sector.
  • Demonstrated experience managing a caseload of clients with complex mental health, psychosocial, homelessness, or alcohol and other drug support needs.
  • Strong understanding of recovery oriented, strengths based, and trauma informed approaches, with the ability to support people experiencing significant personal challenges.
  • Excellent communication and relationship building skills, with the ability to establish trust, set clear expectations, and maintain professional boundaries with participants and stakeholders.
  • Well developed administration and technology skills, including experience with case notes, reporting, data management systems, and Microsoft Excel.
  • Proven ability to work independently, exercise sound judgement, prioritise competing demands, and make informed decisions in a fast paced environment.
  • Mental Health First Aid, suicide intervention training, trauma informed care training, or similar professional development will be highly regarded, along with a genuine commitment to participant wellbeing and recovery.
  • First Aid & CPR Certificate.
  • Full driver licence with no restrictions.
  • Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
