Job summary

Flourish Community Solutions in Toowoomba, QLD is seeking an experienced Case Manager or Family Practitioner to work directly with young people and families in home and community settings. You will provide therapeutic, strengths-based support, and help families access services while maintaining thorough case notes and promoting safe, stable outcomes.

Flexible casual hours with afternoon/evening shifts and some weekend work are available.

Flourish Community Solutions - Toowoomba QLD

You will work directly with young people and families in their homes and communities, supporting meaningful and lasting change. This is hands-on, relationship-based work that requires empathy, consistency, and a strong therapeutic approach.

We are specifically looking for those with good availability; covering 2 afternoon/evening shifts and 1 weekend shift across Saturday and Sunday on average. And other additional shift patterns on offer. Only experienced Case Managers or Family Practitioners with good availability need to apply. We work under the SCHADS award, and weekend rates apply, making this an attractive opportunity.

Key responsibilities

• Supporting family contact and connection
• Assisting with life skills and daily living
• Providing parenting support and mentoring
• Supporting emotional regulation and behaviour management
• Implementing strategies to address concerns
• Helping reduce crises and build stability within families
• Provide therapeutic, strengths-based support to families and young people
• Work with clients experiencing child safety involvement, mental health concerns, trauma concerns, DFV, AOD, neurodiversity, and complex behaviours
• Support families with daily living and community access
• Maintain accurate case notes and documentation

About you

• Experience working with complex young people and/or families
• Can remain calm, responsive, and supportive in crisis situations
• Deep understanding of trauma-informed practice
• Ability to problem-solve and see opportunities in crisis
• Extensive understanding and ability to complete case notes
• Ability to identify, manage, and report risk appropriately
• Strong professional boundaries and communication skills
• Confidence working both independently and within a team
• Commitment to culturally safe and respectful practice
• Current Blue Card / Working with Children Check
• Current Driver's Licence and reliable vehicle
• Smartphone and computer access
• Desirable: Tertiary level qualification in the relevant field
• Considered: Certificate IV or Diploma with more than 2 years of experience in Community Services, Youth Work, Mental Health, Social Work / Social Welfare, or Family Intervention or similar
• Demonstrated experience in therapeutic, residential care, or support-based roles
• Understanding of child protection concerns and risk factors
