Job summary

HOPE Support Services seeks a dedicated Case Manager to lead a team of Youth Workers supporting Young People in residential placements and in the community. You will oversee daily operations, provide staff supervision and development, and ensure trauma-informed therapeutic care within safe, hygienic environments.

Successful applicants must hold a Certificate IV or higher in Youth Work or related Community Services field, plus a Management Qualification and current Blue Card/Working with Children

HOPE Support Case Managers lead a team of Youth Workers who support Young People in our residential placements and in the community. We support Young People from diverse backgrounds, ages, and are experienced with dealing with complex to extreme behaviours.

As a HOPE Support Case Manager, responsibilities and duties include (but are not limited to):

Work a variety of shifts at the Office and in our homes - weekday, afternoon,weekend hours, sleepovers, and on call as per case needs

Manage and oversee the day to day running of the residential placement

Provide leadership and management to all staff including supervision and professional development

Ensure that the Team provides trauma informed therapeutic care to young people within a safe, clean and hygienic environment

Manage positive relationships with all stakeholders

Ensure the Team adheres to all reporting and documentation requirements

Write Tailored Support Plans for young people

Attend critical appointments with young people and facilitate/attend relevant Team and Stakeholder meetings

Complete rosters that reflect smart rostering and industrial compliance

Plan and conduct developmentally appropriate individual and group activities consistent with meeting the young people's needs and goals in line with Case Plans and Tailored Support Plans

Contribute to efficient and comprehensive recruitment, induction, and training of new staff

Implement principles and strategies to role model, mentor and teach young people necessary life skills

Maintain professional conduct along with HOPE values at all times

We are currently looking to hire Case Managers to fill positions in our residential placements in the following regions:

Gold Coast and surrounding areas

Brisbane and Surrounding areas

Logan and surrounding areas

Moreton Bay Region including Caboolture and surrounding areas

**Successful Applicants require the following:

Current Blue Card (Working with Children Check)

Child Safety and Personal History Screening Check

Must have minimum Certificate IV or higher in Youth Work or Community Services related industry

Must have relevant Management Qualification

Current First Aid Certificate

Minimum of 12 months experience in Case Management (residential setting), or similar industry

Current valid driver's license

Strong written and verbal communication skills

Awareness and understanding of Child Protection Act, Charter of Rights, Standards of Care

Ability to build rapport quickly and maintain relationships

Ability to work under pressure and deliver results

Ability to work a range of shifts including sleepovers, weekend work, and on call
