Job summary

Life Without Barriers is seeking a Case Coordinator in the National Immigration Support Service to provide personalised intake and triage support for asylum seekers and refugees in the Dandenong area.

You will perform comprehensive assessments, determine eligibility, assist with SRSS applications, and connect clients with holistic supports. The role requires culturally responsive practice and collaboration with diverse partners.

Qualifications

• Client intake and case coordination experience.
• Ability to assess needs and develop/review case plans.
• Facilitate supports for independent living and service continuity.
• Experience with complex physical/mental health clients.
• Understanding of casework practices for CALD clients.
• Multilingual communication skills are advantageous.

Responsibilities

• Conduct thorough intake assessments to determine eligibility and immediate needs.

• Refer clients to appropriate community supports and services; coordinate pathways.
• Conduct risk assessments, develop safety plans, and maintain accurate records.

• Collaborate with internal and external stakeholders; represent the program.
• Deliver presentations and information sessions to external organisations.

About the Role

Employment Type:Permanent

Attendance:Full Time, 38 hours per week.

Office Location:DandenongVIC, with flexibility to work from office and home.

As a Case Coordinator within the Intake team, you will provide personalised, responsive, and high-quality assessment and triage support to clients. This role is critical in ensuring timely access to services and building strong pathways for support.

In this role, you will:

• Conduct thorough intake assessments to determine client eligibility and immediate support needs
• Work collaboratively with clients to prepare, complete, and submit SRSS applications accurately and in a timely manner
• Assist clients with referrals to appropriate community supports and services, ensuring coordinated and holistic support pathways
• Ensure client safety by undertaking risk assessments, developing safety plans, and maintaining accurate, compliant, and timely records
• Build strong, trust-based relationships with clients and their families, using strengths-based, culturally responsive, and trauma-informed approaches
• Engage and collaborate with internal and external stakeholders, including conducting outreach assessments and representing the program within the community
• Deliver presentations and information sessions to external organisations to increase awareness and understanding of NISS supports and referral pathways

Skills & Experience

• Relevant qualification in a Human Services related field and/or equivalent experience.
• Client intake and case coordination experience, with demonstrated ability to assess needs, develop and review case plans, and facilitate supports that promote independent living and service continuity.
• Ability to perform comprehensive assessments in areas such as finances, health, housing, social needs, vocational skills, and education.
• Experience managing clients with complex physical and mental health issues
• Working understanding of current casework practices for Culturally and linguistically diverse clients.
• Multilingual communication skills would be highly regarded
