This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through in our Carer Gateway Hub.
The Carer Services Navigator role is based in the Toowoomba region, providing consistent presence and reliable support to carers however adopts a 'no wrong door' approach. Working in collaboration with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to further strengthen and embed carer support locally.
To create seamless connections between Carers and vital community resources, cultivating meaningful relationships that support wellbeing and independence. Supporting Carers to thrive, focusing on holistic well-being, going beyond basic needs to help carers achieve their full potential. The service is Carer focused, ‘how can we help’ and a ‘no wrong door approach’. Oversee the Toowoomba Carer Hub, facilitating social interaction and promoting wellbeing, connection and a sense of belonging.
