Job summary

Ballarat Community Health is seeking a Care Partner - Case Management for their Support at Home program in Ballarat, VIC. The role is 0.8–1.0 FTE with a salary range of AU$33.98–AU$50.25 per hour, depending on skills and experience.

You’ll support clients to live independently at home by coordinating services, reviewing care plans, and helping navigate complex appointments. BCH values inclusive, person-centred care and flexible arrangements.

Qualifications

• Experience in care management, case management or care coordination.
• Strong understanding of person-centred and consumer-directed care.
• Confidence using My Aged Care and digital case management systems.
• Strong communication, organisation and problem-solving skills.
• Experience supporting older people, including those with more complex needs.

Role: Care Partner - Case Management (Support at Home Program)

Time fraction: Part-time - Full time: 0.8 to 1.0 FTE

Salary: $33.98 - $50.25 depending on skills and experience.

What if your workday was filled with moments that truly matter?

Joan wants to keep living independently at home. She loves her garden, catching up with friends and doing things for herself. But since giving up driving, getting to medical appointments has become harder. Joan contacts My Aged Care, completes an assessment and is allocated a Support at Home program

That's where you come in.

As Joan's Care Partner, you listen to what's getting in the way and work with her to find a solution. Together, you'll adjust her services to include transport and some extra support to help her navigate those confusing specialist appointments.

The relief that brings Joan? That's what being a Care Partner is all about.

As a Care Partner, you'll build relationships, solve problems and coordinate the care and services that help older people stay independent at home.

Your day as a Care Partner

No two days or people will look quite the same.

• sit down with someone to understand what's important to them and what they want from their support
• develop and review care plans as people's needs and goals change
• coordinate services, supports and appointments
• work with families, carers and other professionals to solve problems
• check in to make sure services are working as they should
• help someone understand their options and make informed choices about their care
• manage care budgets and make sure available funding is being used in ways that support each person's goals
• work alongside a Clinical Care Partner when someone's needs become more complex
• navigate My Aged Care and case management systems while keeping the person at the centre of it all.

What we're looking for

You’ll bring:

• experience in care management, case management, care coordination or a similar role
• a strong understanding of person-centred and consumer-directed care
• confidence using My Aged Care and digital case management systems
• strong communication, organisation and problem-solving skills
• experience supporting older people, including people with more complex needs.

Experience with dementia, palliative care, trauma-informed practice or cultural safety? Even better!
