Job summary

LDK Seniors’ Living is seeking a Care Partner to coordinate resident care and service requirements, working closely with residents or their representatives to ensure needs and wellbeing are met. You will support pre-entry clinical assessments, move-in support, care planning, delivery and evaluations, referrals and documentation of externally funded care.

As a Care Partner you’ll liaise with a wide range of internal and external stakeholders, sharing knowledge to support staff and promote

Qualifications

• AHPRA registration is required.
• Bachelor of Nursing is required.
• 2 years of experience working in a clinical setting.
• Experience in the community care sector/with home care packages.
• Experience working as a member of a multidisciplinary team.
• Aged care nursing experience is desirable.
• Experience in a team lead or management position is desirable.

Responsibilities

• Coordinate the delivery of safe, high-quality, evidence-based care through assessments, planning and evaluations that are person-centred and aligned with regulatory and organisational standards.
• Build and maintain strong relationships with residents and families.
• Interpret care options and funding arrangements for residents.
• Connect residents to health and other services as needed and collaborate with GPs, nurse practitioners, pathology services, allied health professionals, hospitals, pharmacies and government agencies.
• Liaise with rostering to ensure services are delivered as planned and in a timely manner.
• Ensure timely and accurate execution of documentation in line with policy and procedures.
• Complete Move In assessments and care planning.
• Provide daily, weekly and monthly updates to the Clinical and Assistant Care Manager.
