Job summary

New Home Care Pty Ltd is seeking a dedicated Care Partner to join our growing aged care team in Sydney. You will support a portfolio of clients, coordinating services with families and service providers to ensure safe, dignified home care.

Mandarin and/or Cantonese language skills are highly regarded, and experience in Support at Home or Home Care Packages is beneficial. This is a hands-on role with teamwork across multidisciplinary services.

Qualifications

• Experience in aged care, home care, care coordination or case management is required.
• Strong communication and relationship-building skills are essential.
• Good organisational and time-management skills, with ability to manage a client portfolio independently.
• Professional, respectful, and person-centred approach when working with clients and families.

Responsibilities

• Build strong relationships with clients and their families.
• Coordinate and review services based on individual needs and goals.
• Support care planning and ongoing service arrangements.
• Monitor client funding and service utilisation.
• Liaise with support workers, allied health professionals and other service providers.
• Complete regular client reviews and maintain accurate documentation.

This role is suited to someone with previous experience in aged care, home care, case management or care coordination, who enjoys building strong relationships with clients and families and coordinating quality services.

About the Role

Your key responsibilities will include:

• Supporting care planning and ongoing service arrangements

About You

We are looking for someone who has:

• Previous experience in aged care, home care, care coordination or case management
• Strong communication and relationship-building skills
• Good organisational and time-management skills
• Confidence managing a client portfolio independently
• Good computer and documentation skills
• A professional, respectful and person-centred approach
• Ability to work effectively with clients, families and multidisciplinary teams
• Experience with Support at Home or Home Care Packageswill be highly regarded.

Language Skills

Due to our client base, candidates who can communicate in Mandarin and/or Cantonesewill be highly regarded.

Strong English communication skills are also required.
