Churches of Christ on the Gold Coast is seeking a Care Partner in Home Care. You will support older Australians to live independently at home, coordinating services, reviewing care plans and managing budgets with a compassionate, person-centered approach.

The role requires experience in aged care settings, knowledge of relevant standards, and a commitment to integrity, compassion, excellence and courage. Immediate start and permanent, full-time employment on the Gold Coast are offered.

Competenze

• Experience in care coordination, case management, home care, aged care, health or disability services.
• Knowledge of Support at Home, CHSP, aged care quality standards and participant rights.
• Strong care planning, coordination, communication and risk-management skills.
• Confidence managing participant budgets and responding to changing or complex needs.
• Competence using digital care management systems and Microsoft Office.
• Current driver’s licence with access to insured vehicle; National Police Certificate or ability to obtain one.

About the Opportunity

As a Care Partner with Churches of Christ Home Care on the Gold Coast, you'll play an important role in helping older Australians remain independent, connected and living well in their own homes.

What you'll be responsible for

About you

You're a compassionate and organised care professional who builds strong relationships and understands the importance of supporting people to remain independent in their own homes.

You’ll ideally bring:

• Experience in care coordination, case management, home care, aged care, health or disability services.
• Knowledge of Support at Home, CHSP, aged care quality standards and participant rights.
• Strong care planning, coordination, communication and risk-management skills.
• Confidence managing participant budgets and responding to changing or complex needs.
• Competence using digital care management systems and Microsoft Office.
• Certificate III in Individual Support or equivalent, current First Aid Certificate, and relevant medication support and healthy body systems competencies.
• Current driver's licence with access to a comprehensively insured vehicle, and a current National Police Certificate or ability to obtain one.
• Ability to meet relevant organisational vaccination requirements.

Experience with My Aged Care, dementia, complex ageing or palliative care will be highly regarded.
