Job summary

Carer’s Network in Bendigo seeks a dedicated Care Partner to manage client relationships, assess needs, and build personalised in-home care plans. You will develop budgets, manage service agreements, and coordinate with a wide team to deliver high-quality support in the community.

The role emphasises relationship-led care, ownership, and the ability to adapt in a changing environment. A dip or higher in aged care or related field is preferred with a Victorian Driver Licence and NDIS screening.

Qualifications

• Diploma or tertiary qualification in aged care, community services, disability, social work, nursing, allied health, or a related field.
• NDIS Worker Screening Check.
• Victorian Driver Licence.
• 1 year (Required) — relevant case management experience in home-based aged care or community services.

Responsibilities

• Client relationships: be the primary contact for clients and families from referral and assessment through ongoing reviews.
• Care planning: conduct in-home assessments and build personalised plans around client strengths, priorities and goals.
• Budgets and agreements: develop individual budgets, manage service agreements, and monitor package utilisation.
• Service coordination: partner with Scheduling, care workers, allied health professionals and stakeholders for seamless support.
• Quality and compliance: maintain accurate records and ensure services meet Aged Care Quality Standards and regulatory requirements.
• Better ways of working: identify opportunities to strengthen client experience and improve service delivery.

What you will own

• Know community care.You bring at least one year of relevant case management experience in home‑based aged care or community services; deeper experience is highly regarded.

• Bring relevant training.A diploma or tertiary qualification in aged care, community services, disability, social work, nursing, allied health, or a related field is highly regarded. A nursing background is desirable.
• Can travel locally.You hold a valid Victorian Driver Licence.
• NDIS Worker Screening Check.You have or are willing to obtain the required NDISWSC clearance.
