Job summary

VMCH in Victoria is seeking a Care Partner based in Shepparton or Wangaratta to support older Australians at home and in the community. You will build real relationships, empower clients to stay independent, and coordinate personalised services across the Hume region.

This role requires travel to visit clients, assess needs, develop care plans, manage budgets, and work with families and care networks. You will use Microsoft Office and client management systems, and uphold aged care standards to

Qualifications

• Diploma qualification in a relevant field or higher.
• Experience in case management, care coordination or client services.
• Experience completing assessments, developing care plans and supporting clients with complex needs.
• Ability to manage consumer budgets and support services effectively.
• Sound computer skills, including Microsoft Office and client management systems.
• Excellent organisational skills with the ability to prioritise and work autonomously.
• Understanding of community aged care services, referral pathways and person-centred care principles.
• Knowledge of aged care legislation, quality standards and sector reforms.
• Current Victorian Driver's Licence & car registration.
• Willingness to travel across regional Victoria.
• National Police Check.

What’s required

• Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
• Experience in case management, care coordination or client services within aged care, community services or a similar environment
• Experience completing assessments, developing care plans and supporting clients with complex needs
• Ability to manage consumer budgets and support services effectively
• Sound computer skills, including Microsoft Office and client management systems
• Excellent organisational skills with the ability to prioritise and work autonomously
