Job summary

Right at Home Southwest Victoria is seeking a Care Partner to manage a portfolio of clients with Monday to Friday hours, no evenings or weekends, and a focus on building meaningful relationships through person-centred care.

You will develop care plans, conduct routine reviews, and work with clients and their representatives to ensure responsive, coordinated services, supported by the local clinical care team.

Qualifications

• Qualification in Enrolled Nurse/Endorsed Enrolled Nurse (registered with AHPRA), Social Worker, Occupational Therapy or Cert III/IV in Individual Support (with previous experience as a Care Partner).
• Current Driver Licence.
• National Police Check.
• Current First Aid and CPR.

Responsibilities

• Managing an allocated portfolio of clients.

• Completing routine client reviews and follow-up.
• Building strong relationships with clients and their representatives.
• Identifying changes in client needs and coordinating appropriate responses.
• Working collaboratively with the Care Manager and Clinical Care Team Leader.

• Previous knowledge and experience of Support at Home/Home Care Program.

About You

You'll also have:

About the Role

As a Care Partner, you'll become the primary point of contact for an allocated portfolio of clients, working closely with clients, representatives and the wider Clinical Care Team to ensure services remain responsive, coordinated and focused on achieving positive outcomes.

Your responsibilities will include:
