Job summary

VMCH in Victoria is seeking a Care Partner based in Shepparton or Wangaratta to empower older adults to remain independent at home. You will coordinate personalised services across the Hume region, with regular travel to build relationships and support networks.

Bring a Diploma or higher and experience in case management or care coordination. You will work with clients to develop care plans, manage budgets, and use client management systems, while upholding person-centred care.

Qualifications

• Diploma qualification or higher in a relevant field.
• Experience in case management or care coordination in aged care or community services.
• Experience developing care plans and supporting clients with complex needs.
• Ability to manage consumer budgets and support services.
• Proficient with Microsoft Office and client management systems.
• Excellent organisational skills and autonomy.
• Knowledge of community aged care services and referral pathways.
• Understanding of aged care legislation and quality standards.
• Current Victorian Driver's Licence and car registration.
• Willingness to travel across regional Victoria.
• National Police Check.

What’s required

• Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
• Experience in case management, care coordination or client services within aged care, community services or a similar environment
• Experience completing assessments, developing care plans and supporting clients with complex needs
• Ability to manage consumer budgets and support services effectively
• Sound computer skills, including Microsoft Office and client management systems
• Excellent organisational skills with the ability to prioritise and work autonomously
• Understanding of community aged care services, referral pathways and person-centred care principles
• Knowledge of aged care legislation, quality standards and sector reforms
• Current Victorian Driver's Licence & car registration
• Willingness to travel across regional Victoria
• National Police Check
