Job summary

Uniting NSW & ACT is seeking a permanent full-time Care Partner to coordinate person-centred home and community care for a portfolio of clients, helping them live safely at home.

You will build trusted relationships, develop and review care plans within funding, and act as the central contact for clients, families and providers.

Travel between Uniting community sites and client locations as required, and contribute to quality improvement and record-keeping using client management systems.

Qualifications

  • Certificate IV in Aged Care or Diploma of Community Services.
  • A diploma or bachelor qualification in aged care, healthcare or business is highly regarded.
  • Experience coordinating care, managing a client portfolio or working in community-based services.
  • Strong relationship-building, communication, organisation and problem-solving skills.
  • Confidence using client management systems and maintaining accurate records.
  • Current Australian driver’s licence, access to your own vehicle and comprehensive car insurance.
  • Current First Aid Certificate.

Responsibilities

  • Maintain accurate records and confidently use client management systems and other practice tools.
  • Travel between Uniting community sites and client locations as required.

About the Role:

As a Care Partner, you will be the trusted point of contact for a portfolio of clients, coordinating person-centred home and community care that reflects their goals, preferences and funding. You will build strong relationships, develop responsive care plans and bring together the right services so clients can remain safely at home and connected to their community for longer.

Your Key Accountabilities:
