Job summary

Right at Home NQ is seeking an experienced care professional to conduct client assessments, develop care plans, and coordinate in-home services.

Diploma of Nursing with AHPRA registration (preferred) and Certificate IV in Ageing Support. Experience in community aged care, a current driver licence, and up-to-date police check and First Aid/CPR are required. Join a supportive team delivering high-quality and compliant care.

About you

Diploma of Nursing with current AHPRA registration (non-essential).

Certificate IV in Ageing Support, Individual Support or a related qualification (minimum).

Previous experience in community aged care, disability services or home care.

Sound understanding of the Australian Government aged care funding environment, including Support at Home.

Experience with case management, care planning and quality and compliance requirements.

Current Driver Licence and access to a reliable, comprehensively insured vehicle.

Current National Police Check.

Current First Aid and CPR Certificate.

Excellent organisational, communication and time management skills.

Strong problem-solving abilities and sound clinical or care coordination judgement.
