Job summary

Bolton Clarke Residential Aged Care Opportunities is seeking a dedicated Care Partner in Brisbane to plan and oversee care delivery for older adults living at home or in facilities. You will work with clinical teams to ensure safe, person-centred care and timely documentation in line with program standards.

The role emphasises care coordination, communication with families and healthcare providers, and maintaining quality records while supporting diverse participant needs in a not-for-profit

Qualifications

• Cert III in a relevant aged care field or Diploma-level nursing/care management
• Experience coordinating care in aged care, health or disability services
• Strong understanding of Support at Home Program and aged care rights
• Excellent communication, empathy and cultural competence
• Ability to interpret clinical guidance and escalate appropriately

Responsibilities

• Collaborate with participants to develop individualised care plans.
• Liaise with GPs, specialists and allied health providers for integrated care.
• Review care notes and service data; identify risks or changes in needs.
• Maintain accurate, timely care notes and documentation to support outcomes.
• Support participants and their carers to make informed service decisions.
• Monitor spending to align resource allocation with changing needs.

The Care Partner is the designated care manager under the Support at Home Program, responsible fordelivering care management activities that enable older people to live safely and independently athome.You will play a key role in planning and overseeing care service delivery across all ongoingsupport classifications within scope and work closely with the appropriate clinical team memberswhen a participant’s needs require clinical input or complex care planning.

As a Care Partner, you will:

• Collaborate with participants to develop individualised care plans. Incorporating the participant’s goals, service preferences, cultural needs, risk factors, and AT-HM requirements.
• Collaborate with participants to develop individualised care plans.
• Liaise with general practitioners, specialists, and allied health providers to ensure integratedcare.
• Review care notes and service delivery data; proactively identify and address emerging risks orchanges in participant needs.
• Consistently meet quality compliance and audit requirements, ensuring accurate and up-to-date recordsare maintained.
• Facilitate or participate in case conferences, ensuring care plans reflect shared decision-making and supported risk-taking.
• Monitor spending to prevent underspends and overspends and align resource allocation withchanging needs.
• Maintain accurate, timely care notes and ensure documentation supports outcomes under theStrengthened Aged Care Quality Standards.
• Ensure care management claims comply with Support at Home business rules, includingexclusion of administrative, rostering or travel tasks.
• Supporting participants (and their carer or supporter) to make informed service decisions.

To be successful, you will have:

• Cert III Individual Support, or Diploma-level qualification in Nursing, Community Services, Aged Care, Case Management, or equivalent experience.
• Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services.
• Strong understanding of the Support at Home Program, Quality Standards, and aged care rights.
• Strong communication, empathy and cultural competence.
• Competence in interpreting clinical guidance and escalating appropriately.
