Job summary

Churches of Christ Queensland are seeking an experienced Care Manager for the Warwick Aged Care Service, a modern 128-bed facility in Warwick, Queensland. This permanent full-time role involves clinical leadership, guiding nursing and care teams, and driving quality and resident outcomes.

You will oversee assessments, care planning, governance, and compliance, while fostering a compassionate, resident-centred culture.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• 3–5 years post-graduate experience in aged care or acute clinical settings.
• Strong knowledge of contemporary aged care practice and quality standards.
• Excellent communication, leadership, decision-making and problem-solving skills.
• Experience with AN-ACC and aged care funding processes.

Responsibilities

• Lead, support and develop nursing and care teams.
• Oversee resident assessments, care planning and clinical documentation.
• Monitor clinical practice, care outcomes and quality indicators.
• Coordinate care with internal teams and external providers.
• Review AN-ACC information to support submissions.
• Ensure programs, policies and procedures meet regulatory requirements.
• Support incident management, risk mitigation and continuous improvement.
• Build relationships with residents, families and healthcare professionals.
• Promote a collaborative, resident-focused team culture.
