Job summary

Opal HealthCare is seeking a Care Manager to join our leadership team in delivering exceptional clinical care and leading high-performing teams. You will oversee clinical governance, workforce capability and operational performance across the Warrnambool Place Care Community.

Reporting to the General Manager, you will align care delivery with Aged Care Act 2024, Aged Care Quality Standards, and AN-ACC, while actively mentoring staff and driving improvements in resident outcomes.

Qualifications

• Registered Nurse with AHPRA registration (minimum 3 years).
• Demonstrated experience leading clinical teams in residential aged care.
• Understanding of Aged Care Quality Standards and AN-ACC.
• Ability to lead change, manage risk, and improve outcomes.
• Visible, values-led leader with strong mentoring capability.

Responsibilities

• Lead clinical care delivery ensuring compliance with Aged Care Quality Standards and regulatory obligations.
• Oversee care planning, admissions reviews, and changes in resident condition.
• Monitor and analyse clinical indicators, incidents and risk trends.
• Apply evidence-based practice to improve outcomes.
• Build relationships with GPs, hospitals and allied health providers.
• Ensure adherence to Aged Care Act 2024 requirements and reporting obligations.
• Drive continuous improvement, audit readiness, and clinical excellence.
• Lead compliance with incident management and accreditation standards.
• Foster a values-led, high-performing culture.
• Oversee education, onboarding and competency development.

About the Role

Reporting to the General Manager, you will lead the clinical and care workforce to deliver safe, high-quality, person-centred care aligned to the Aged Care Act 2024, Aged Care Quality Standards, and AN-ACC requirements.

This is a hands-on leadership role, balancing clinical governance, workforce capability, and operational performance.
