Job summary

The Residential In-Reach (RIR) program at GV Health provides a visiting service to Residential Aged Care Facilities in the Eastern Hume region. The Care Coordinator delivers specialist assessment, clinical expertise and short-term management to optimise resident journeys and prevent avoidable ED visits.

You will provide clinical consultancy to RACF staff and collaborate with the broader team to support residents and families across the care continuum.

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse.
• Senior level work experience in Aged Care and/or Emergency nursing, acute care nursing.
• Broad postgraduate clinical experience in comprehensive assessment of older adults.
• Ability to work collaboratively within a team environment and maintain professional relationships.

Responsibilities

• Provide specialist assessment, clinical expertise and short-term management for RACF residents to improve patient journeys and prevent ED presentations.
• Offer clinical consultancy to RACF nursing staff and care staff and to relevant community agencies.
• Support implementation of clinical management plans across the care continuum.
