Job summary

St Vincent's Private Hospital Toowoomba is seeking a Clinical Nurse Consultant for Residential In Reach in Melbourne, VIC. The role focuses on clinical assessment and management for RACF and community residents to reduce ED presentations, while providing expert nursing consultancy.

You will support RACF staff with advanced assessment, care planning, and care coordination, ensuring effective outcomes and timely discharges.

Qualifications

• Current nursing registration with AHPRA.
• Postgraduate nursing training in relation to aged care or acute/critical care management, or working towards.
• Advanced skills in comprehensive assessment of the older person.
• Excellent communication and leadership skills.
• Current Victorian Driver's License or equivalent.

Responsibilities

• Provide comprehensive assessments, care planning, coordination and treatment for RACF and community residents who are experiencing medical issues.
• Provide clinical advice to RACF staff, patients, carers and other health care professionals and evaluate treatment outcomes.
• Triage and screen referrals with a focus on collaboration and care planning across at-home services.
• Support timely discharge of RACF residents from ED/inpatient units and ensure follow-up.
