Job summary

Lumia Care is seeking an experienced Care Coordinator to join our Adelaide team. You will coordinate Support at Home services, leverage CHSP/CHIS funding knowledge and work with Care Partners and frontline teams to deliver compliant, client-centered care.

Strong communication, budget awareness and rostering system experience are essential as you manage client records, service agreements and onboarding while prioritising safety and quality in a busy aged care environment.

Qualifications

• Hands-on experience in Support at Home or CHSP environments.
• Strong knowledge of CHSP/CHIS funding and aged care service requirements.
• Experience in a Care Coordinator, Service Coordinator, Client Services or similar role.
• Strong understanding of care plans, budgets, client records and compliance.
• Excellent communication and stakeholder management skills.
• Ability to manage competing priorities in a fast-paced aged care setting.

Responsibilities

• Coordinate Support at Home services and ensure timely, quality care.
• Apply CHSP/CHIS funding knowledge to day-to-day coordination.
• Maintain client records, care plans, service agreements and documentation.
• Coordinate client onboarding, service commencement and changes to care arrangements.
• Monitor service delivery, funding utilisation and client needs.
• Work with Care Partners, Team Leaders and frontline teams to resolve issues.
• Ensure services and documentation meet compliance and quality requirements.
• Provide responsive and professional support to clients, families and stakeholders.
• Manage competing priorities in a fast-paced aged care environment.
