Job summary

Teachers Health is seeking a Care Coordinator to work with members, healthcare providers and multidisciplinary care teams to support individuals managing chronic health conditions or requiring hospital substitute treatment. You will coordinate care, provide education and support, and help members navigate their healthcare journey to achieve better health outcomes.

Qualifications

• Tertiary qualification in Nursing, Allied Health or a related health discipline.
• Minimum two years' post-qualification clinical experience.
• Current registration with the relevant professional association.
• Experience delivering services via telehealth.

Responsibilities

• Identify and engage eligible members through referrals, enquiries and outreach programs.
• Support the assessment, development and monitoring of personalised care plans.
• Coordinate care with members, healthcare providers, service providers and broader care teams.
• Provide health education, guidance and support within your clinical scope of practice.
• Maintain accurate clinical records and contribute to quality care outcomes.
• Participate in clinical audits, professional development and continuous improvement initiatives.
• Contribute to a positive, safe and member-focused team culture.

What will you do?

• Experience in care coordination, chronic disease management or health coaching.
• Experience delivering services via telehealth.
• Strong customer service experience within a healthcare environment.
