About Ableconnect
Ableconnect is a registered NDIS provider committed to delivering safe, reliable and person-centred supports that empower people with disability to live meaningful lives, achieve their goals and exercise choice and control.
We provide a range of supports to NDIS participants, including Supported Independent Living (SIL), Specialist Disability Accommodation (SDA)-related support services, in-home support and community participation.
At Ableconnect, we believe quality care starts with understanding each participant as an individual. We are committed to building trusted relationships, supporting independence and delivering services that reflect each participant’s needs, preferences and aspirations.
About the Role
We are seeking an experienced, organised and proactive Care Coordinator to join our growing team.
Reporting directly to the CEO, you will play a key role in coordinating and overseeing the day-to-day delivery of supports across our SIL and community-based services. You will work closely with participants, support workers, families, allied health professionals and other stakeholders to ensure supports are delivered consistently, safely and in accordance with individual support requirements.
The role combines participant coordination, staff supervision, rostering, recruitment, compliance monitoring and continuous service improvement. You will be expected to take ownership of operational responsibilities, identify opportunities to improve service delivery and work collaboratively with the wider team to support Ableconnect’s ongoing growth.
