Job summary

Allinto is seeking two Care Coordinators to join the Ageing Services Team, supporting CHSP clients across Queensland. The roles are full-time based in Eight Mile Plains or Morayfield, with hybrid options after probation, on a contract up to 30 June 2027 with possible extension.

You will manage a caseload and deliver person-centred care that promotes independence and dignity, working with clients, families and providers to ensure high-quality support.

Qualifications

• Prior experience in a similar role (aged care, health, or community services).
• Knowledge of the Aged Care Act, CHSP Guidelines, and Aged Care Quality Standards (or willingness to learn).
• Excellent communication, coordination, and relationship-building skills.
• Empathy, cultural sensitivity, and a genuine commitment to improving the lives of older people.
• Strong documentation, time management, and organisational skills.

Responsibilities

• Support clients with care services that meet aged care standards and program guidelines.
• Organise client intakes, do assessments, and create care plans that promote wellbeing and independence.
• Keep track of client needs and adjust support when things change, referring to other services if needed.

About the role

This is a full-time position based in either Eight Mile Plains or Morayfield locations with some hybrid opportunities after probation on a contract up to end of June 2027 with opportunity to extend. We’re seeking 2 Care Coordinators to join our growing Ageing Services Team. In this role, you’ll support ageing Australians to live with dignity, independence, and connection by managing a caseload of Commonwealth Home Support Program (CHSP) clients.

You’ll coordinate holistic support services, conduct assessments and reviews, and build trusted relationships with clients, families, and providers. Delivering care that is meaningful, respectful, and tailored to individual goals.

Responsibilities include:

• QLD Blue card (working with children’s check)

• Relevant qualification in Community Services, Aged Care, or similar (desirable)
