Job summary

Mercy Health in Barwon is seeking a Care Partner to join our for-purpose team. The role focuses on delivering compassionate community and in-home health support, using strong organisational and communication skills to coordinate care and enable re-enablement for clients.

You'll work across a flexible casual arrangement (15–38 hours per week), with opportunities to develop experience in nursing or allied health while upholding safety and quality standards.

Qualifications

• Experience in service delivery to clients living in the community is desirable.
• Experience in Nursing or Allied health is desirable.

Responsibilities

• Coordinate care and support for clients to enable re-enablement.
• Provide compassionate patient care and management in a community setting.

About the opportunity

At Mercy Health, you'll become part of a proud history built on over 100 years of expertise, skill and care. As a Care Partner you'll use your outstanding organizational skills, budget management and customer service skills as well as strong interpersonal, communication and leadership skills.

Experience in service delivery to clients living in the community, assessing, referring and supporting re-enablement for clients is desirable as is experience in Nursing or Allied health.
