Job summary

Mercy Health in Barwon is seeking a Care Partner on a casual basis to support community-based client care. You will use strong organisational, budgeting and customer service skills to deliver high-quality care and build trusted relationships with clients and families.

Experience delivering services to community clients, assessing, referring and supporting re-enablement is desirable, with Nursing or Allied health background preferred. Police check and NDIS screening may be required.

Qualifications

• Experience in service delivery to clients living in the community, assessing, referring and supporting re-enablement for clients is desirable.
• Experience in Nursing or Allied health is advantageous.
