Job summary

Apunipima Cape York Health Council – Cairns QLD is seeking an ITC Care Coordinator to travel among Cape York communities, facilitating access to a chronic care program for eligible clients. You will work under the ITC Program Lead, with pre-employment vaccination screening required for the successful applicant.

The role requires mandatory checks (Blue Card, driver licence, police check) and a Certificate IV or Diploma in Aboriginal and/or Torres Strait Islander Primary Health Care, with a strong

Qualifications

• Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement.
• Current Queensland Drivers Licence.
• National Police Check within the last 3 months or willing and able to obtain prior to commencement.
• Certificate IV or Diploma in Aboriginal and/or Torres Strait Islander Primary Health Care.

Responsibilities

• Travel to various Cape York communities and facilitate access to the program for eligible clients with chronic health conditions.
