Job summary

Western District Health Service in Hamilton, VIC, is seeking Expressions of Interest from current employees for the Cancer Care Nurse within our Cancer Services team. This advanced practice role coordinates care across the cancer journey, acts as a key contact for patients and families, and collaborates with oncologists and multidisciplinary teams to deliver timely, person-centred care.

Applicants must be Registered Nurses with AHPRA, a Bachelor of Nursing, and experience in oncology care;

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Bachelor of Nursing qualification.
• Experience in oncology nursing, cancer services or care coordination.
• Postgraduate qualifications in Oncology Nursing desirable.

Responsibilities

• Provide expert care coordination and navigation throughout the cancer care continuum.
• Act as a key contact for patients, families and carers.
• Coordinate referrals, investigations, appointments and treatment requirements.
• Support patients to navigate often complex healthcare systems and treatment journeys.
• Work collaboratively with oncologists, multidisciplinary teams and external service providers.
• Deliver education and support to patients and carers to promote informed decision-making.
• Provide clinical leadership, guidance and support to colleagues involved in cancer care.
• Contribute to service development, quality improvement and evidence-based practice initiatives.
