MCCGC/CURA Community Services in Southport, Queensland, is seeking a Part-Time Community Pathway Connector to provide culturally safe information and referral support for people from CALD backgrounds. You will help clients navigate mental health services, coordinate with interpreters, and connect them to appropriate resources within the community sector.

You will work with a diverse team across MCCGC and CURA Aged Care Services, bringing strong communication, cultural awareness, and advocacy

Formación

• 1-3 year's experience in a similar position.
• Demonstrated experience and understanding of support coordination.
• Highly developed communication and interpersonal skills to engage with CALD families and communities.
• Extensive knowledge working with CALD communities and people experiencing mental health conditions.
• Current Driver's Licence and Working with Children (Blue Card) with NDIS Worker Screening (Yellow Card).
• National Police Clearance (or ability to obtain one).

Responsabilidades

• Cultural competency by providing sensitive support and advocacy for participants and their needs and collaborating with community organisations through a strong understanding of cultural diversity.
• Improve awareness and knowledge of participants and communities about mental health support and services more broadly.
• Reduce barriers to accessing mental health services by providing assertive outreach to individuals with a disability.
• Client support and navigation by offering one-on-one support to clients.
• Provide tailored information and refer clients to relevant services and resources according to their needs and act as a liaison between the client and service providers to ensure their needs are understood and met.
• Working closely with community agencies, interpreters, and cultural awareness educators to ensure a comprehensive support network for our clients.
