Job summary

360 Health Management is seeking a motivated Behaviour Support Practitioner to join our team in Swan Hill. You will support individuals with challenging behaviours, develop BSPs, and work with families and carers to improve quality of life.

The role requires compassion, resilience, and the ability to travel to client homes or communities within an hour of Swan Hill. An Australian driver’s licence is preferred.

Qualifications

• Degree in psychology, social work, or education or related field.
• Experience or demonstrated ability working with individuals with challenging behaviours.
• Strong communication and interpersonal skills.
• Ability to travel to attend 1:1 meetings and client homes.

Responsibilities

• Assess root causes of challenging behaviours.
• Develop BSPs to support and reduce behaviours of concern.
• Assist families to implement BSPs in daily routines.
• Educate participants, families and carers on behaviour management techniques.
• Monitor progress and adjust BSPs as needed.
• Maintain accurate client progress records and reports.
• Complete timely case notes and participate in care team meetings.
• Travel up to an hour from Swan Hill for meetings.
