Job summary

Aligned Healthcare is seeking a dedicated Behaviour Support Practitioner to deliver high-quality, person-centred services for NDIS participants in a community-based role with travel within a reasonable radius from home.

You will conduct comprehensive behaviour assessments, develop and implement PBSPs, and work with participants, families and a multidisciplinary team to improve quality of life and daily living for diverse client needs.

Qualifications

• Tertiary qualification in psychology, social work, rehabilitation counselling, social sciences or related field.
• Experience conducting behaviour assessments and developing PBSPs.
• Strong knowledge of Positive Behaviour Support frameworks and restrictive practice legislation.

Responsibilities

• Develop, implement, and review Positive Behaviour Support Plans (PBSPs).
• Deliver behaviour support services aligned with participants’ NDIS goals and funding.
• Support participants to increase independence, community inclusion, and quality of life.
• Collaborate with participants, families, carers, Support Coordinators and other professionals.
• Provide evidence-based, least-restrictive interventions under NDIS rules.
