Access Aged Care Nurse role across Cairns and surrounding areas requires an experienced Registered or Enrolled Nurse with strong clinical judgement and coordination skills. You will work within Residential Aged Care Homes, collaborating with GP rounds, specialists and care teams to deliver timely resident care.

The role emphasizes clinical assessment, care coordination and documentation using multiple clinical systems. You will travel between sites and support preventive care with your expertise.

Qualifications

• Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Minimum two years nursing experience post-AHPRA registration.
• Aged care experience or equivalent clinical environment.

Responsabilités

• Coordinate GP and Specialist rounds, including telehealth where appropriate.
• Conduct comprehensive clinical assessments of residents.
• Identify clinical concerns early and escalate appropriately.
• Coordinate referrals, investigations and follow-up care.
• Ensure timely and accurate clinical documentation.
