Job summary

Access Aged Care Nurse role offers flexible part‑time hours on site across Cairns and surrounding areas. You’ll work with GPs, Specialists and care teams to coordinate timely interventions and ensure residents receive appropriate care.

You’ll bring 2+ years nursing experience with AHPRA registration, strong clinical reasoning, and the ability to manage referrals and documentation while using multiple clinical systems. Travel between homes is expected.

Qualifications

• Current AHPRA registration as a Registered Nurse or Enrolled Nurse (RN/EN).
• Minimum two years' nursing experience following AHPRA registration.
• Aged care experience or experience in a relevant clinical environment.
• Strong clinical reasoning and ability to recognise risk.
• Confidence making independent clinical decisions.
• Strong organisation, time management and prioritisation skills.
• Clear, confident communication with multiple stakeholders.
• Confidence using technology and learning new clinical systems.
• Current driver's license and reliable vehicle.
• Flexibility to travel between Residential Aged Care Homes across Cairns and surrounding areas.

Responsibilities

• Coordinate GP and Specialist rounds, both in person and via telehealth.
• Perform comprehensive clinical assessments.
• Identify clinical concerns early and escalate appropriately.
• Coordinate referrals, investigations and follow-up care.
• Ensure agreed clinical actions are followed through.
• Communicate with residents, families, GPs, Specialists and care teams.
• Maintain accurate and timely clinical documentation.
• Use multiple clinical systems and digital tools.
• Build strong relationships with care home teams.
• Support proactive and preventive care to improve resident outcomes.
