Job summary

Palliative Care South East is seeking a Palliative Care Nurse Practitioner to provide autonomous, NP‑led care in home settings and advisory support to residential aged care facilities within our southeast Melbourne region.

You will lead comprehensive assessments, diagnose and plan care with the interdisciplinary team, while mentoring clinical staff and building capacity for a palliative approach across partner sites.

Qualifications

• Nurse practitioner, or working towards, with recent experience in specialist Palliative Care.
• Demonstrated advanced clinical skills, leadership in assessment, treatment and management of Palliative Care and End of Life needs.
• Ability to work within a multidisciplinary framework in varied clinical environments.

Responsibilities

• Provide autonomous NP-led care in clients' homes and advise residential aged care facilities.
• Lead comprehensive assessment, diagnosis, treatment and care planning with the inter-disciplinary team.
• Educate, mentor and support clinical staff to build capability in delivering high-quality palliative care.
