Job summary

Central Highlands Rural Health Service seeks a motivated Registered Nurse to join Macedon Ranges Community Nursing. This district nurse role focuses on delivering comprehensive, home-based clinical care across CHSP, HACC-PYP, DVA, TCP and Support at Home programs.

• Registered Nurse with Australian registration and current practicing certificate.
• Experience delivering community-based nursing care in home settings is desirable.
• Ability to work independently while collaborating with an integrated care team.

Responsibilities

• Provide comprehensive district nursing care in clients’ homes and community settings.
• Manage a diverse caseload across CHSP, HACC-PYP, DVA, TCP and Support at Home programs.
• Deliver person-centred care and support clients to remain healthy and independent at home.
• Collaborate with the wider multidisciplinary team to plan and coordinate care.
