Job summary

Seven Hills region in Australia is seeking an Assistant Care & Services Coordinator to support the delivery of personalised care in Aged Care and Community Services. This hands-on role works with the Care & Services Coordinator to ensure clients receive appropriate care and support based on their individual needs.

You will coordinate care, support the Home Care team, and help develop care plans while building strong relationships with clients and families.

Qualifications

• Certificate III or IV in Aged Care is required.
• Experience in Aged Care, Home Care or Community Care is preferred.
• Strong communication with clients and families and a person-centred approach.

Responsibilities

• Assist with development of individual care plans.
• Coordinate staffing and services around each client’s assessed needs.
• Support staff rostering and allocation.
• Monitor staffing levels and identify training requirements.
• Assist with staff administration and leave management.
• Build strong relationships with clients, families and carers.
• Work with external service providers and health professionals.
• Respond to concerns and escalate issues as needed.
• Support accurate documentation, reporting and compliance.

We’re recruiting an Assistant Care & Services Coordinator on behalf of an established Aged Care and Community Services organisation in the Seven Hills region.

This is a hands‑on coordination role where you’ll work closely with the Care & Services Coordinator to help ensure clients receive the right care and support based on their individual needs.

The opportunity

You’ll play an important role in coordinating care and services for clients, while supporting the wider Home Care team.

Your responsibilities will include:

• Assisting with the development of individual care plans
• Helping coordinate staffing and services around each client’s assessed needs
• Supporting staff rostering and allocation
• Monitoring staffing levels and identifying training requirements
• Assisting with staff administration and leave management
• Building strong relationships with clients, families and carers
• Working alongside external service providers and health professionals
• Responding to concerns and complaints and escalating issues where required
• Supporting accurate documentation, reporting and compliance
• Participating in staff meetings, education and professional development
• Providing weekend support when required
