Job summary

The Department of Justice and Community Safety is seeking an assessment and transition coordinator to help people prepare for life after prison. You will coordinate risk and needs assessments, parole applications and pre-release services, ensuring a smooth transition.

You will work with prisoners, prison staff and service providers to support transition plans, track progress, and coordinate programs and referrals before and after release.
