Job summary

HOBAN Recruitment in Geelong VIC is seeking experienced Allied Health and Health Professionals for APS6 Support Needs Assessor roles. You will conduct support needs assessments, gather and analyse information, and prepare high-quality reports that inform participant outcomes. This role combines face-to-face and virtual assessments within a collaborative team.

Full-time, hybrid work with 37.5 hours per week, ASAP start. Salary is competitive at $61.86 per hour with 12% superannuation.

Job description

HOBAN Recruitment is seeking experienced Allied Health and Health Professionals for APS6 Support Needs Assessor opportunities in GEELONG VIC. This is an exciting opportunity to apply your clinical expertise in a meaningful role that supports people with disability through evidence-based, person-centred assessments. Working within a collaborative environment, you will engage with participants and their support networks, gather and analyse information, and prepare high-quality assessment reports that contribute to positive outcomes.

About the Role
  • Conduct support needs assessments with participants and their representatives.
  • Gather, analyse and synthesise information to support evidence-based assessment outcomes.
  • Interpret clinical and external evidence to assess participant support needs.
  • Prepare clear, accurate and high-quality assessment reports.
  • Communicate assessment processes and outcomes with participants, families and support networks.
  • Apply legislation, policies and procedures while maintaining a participant-centred approach.
  • Collaborate with internal stakeholders to ensure quality and consistency of outcomes.
  • Use digital systems to accurately record assessment information and outcomes.
  • Deliver assessments through both face-to-face and virtual engagement channels.
  • Support a culture of continuous improvement, learning and professional development.

About You

To be successful, you will have:

Mandatory Qualifications

You must hold a recognised tertiary qualification in one of the following:

Allied Health Disciplines
  • Occupational Therapy
  • Physiotherapy
  • Speech Pathology
  • Social Work
  • Dietetics
  • Exercise Physiology
  • Rehabilitation Counselling
  • Counselling
  • Psychotherapy
OR
Relevant Health Disciplines
  • Registered Nurse (RN)
  • Enrolled Nurse (EN)
  • Clinician with an Education degree
  • NDIS Behaviour Support Practitioner

Important: Evidence of your qualification must be included within your CV submission.

Desired Experience & Skills
  • Experience conducting support needs assessments or other person-centred assessment frameworks.
  • Strong commitment to person-centred and strengths-based practice.
  • Excellent interviewing, assessment and stakeholder engagement skills.
  • Experience analysing complex information and making evidence-based decisions.
  • Strong written communication and report writing capability.
  • Understanding of disability, accessibility and inclusive practice.
  • Ability to work within legislative, policy and quality assurance frameworks.
  • Experience using digital case management or assessment systems.
  • Accreditation or experience using the I-CAN Support Needs Assessment Tool will be highly regarded.
