Job summary

St Vincent de Paul Society Queensland in Mackay seeks an AOD Therapeutic Case Manager to champion client-centred recovery through tailored case management strategies. With deep clinical insight and therapeutic expertise, you will empower individuals to actively engage in their journey toward wellbeing.

This permanent part-time role (20 hours per week) is based in Mackay. A Bachelor-level qualification in Community Mental Health or related discipline is required, along with knowledge of AOD

Qualifications

• Understanding of AOD and dual diagnosis treatment principles.
• Experience with assessment, brief intervention and risk assessment.
• Ability to coordinate care and work with diverse stakeholders.

Responsibilities

• Lead and facilitate group module sessions for clients.
• Review case management plans and participate in care coordination meetings.
• Prepare case documentation, referrals and closure summaries.
• Build relationships with internal and external stakeholders to improve service delivery.
• Collaborate with Program Manager to update weekly program to client needs.
