Qualifications

• Approved tertiary qualification in Social Work and eligibility for membership with AASW.
• Demonstrated assessment and intervention options for psychosocial needs.
• Understanding of person-centred care in relation to illness or disability.
• Ability to work independently and as part of an interdisciplinary team.
• Excellent interpersonal, written and verbal communication skills.

Responsibilities

• Provide Social Work services to AACP clients in a community setting.
• Function as a key multidisciplinary team member and plan high-quality interventions.
• Focus on continuous improvement and evaluation of social work outcomes.
• Communicate with GV Health departments and external services to ensure best-practice outcomes.
• Contribute to planning and implementation of patient-centred care across programs.
