Job summary

Silverchain is seeking a Care Partner to support older Australians in Blacktown and surrounds. The role combines community visits with the flexibility to work autonomously from home, empowering clients to stay safe and independent.

You will assess needs, develop personalised Care Plans, coordinate services and advocate for positive health and wellbeing outcomes for clients in their homes. A driver's licence and reliable vehicle are required; experience in community-based case management is

Qualifications

• Experience in community-based case management is highly regarded.
• Experience supporting clients receiving government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Demonstrated empathy, compassion and a client-centred approach.
• Effective problem-solving and critical thinking skills.
• Well-developed organisational and time management skills.
• Ability to work collaboratively across multidisciplinary teams.
• Advocacy skills to support positive client outcomes.

Responsibilities

• Manage a caseload of older Australians receiving government-funded aged care services in their homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor and manage client budgets in line with government program requirements.
• Coordinate services to ensure clients receive the right support at the right time.
• Support new clients through onboarding and ongoing service delivery.
• Build strong relationships with clients, families and key stakeholders.
• Work closely with clinical, scheduling and service delivery teams to deliver coordinated care.
• Advocate for client needs and support positive health and wellbeing outcomes.
