Job summary

Silverchain is seeking a TRANSPAC Care Partner to support older Australians in the North Sydney region. You will assess needs, plan care, coordinate services and advocate for safe, independent living at home, combining community visits with the flexibility to work from home.

This 6-month fixed-term role offers full-time hours (75 per fortnight), autonomy to manage your caseload, and a collaborative team environment with clinical, scheduling and service delivery support.

Qualifications

• Experience in community-based case management is valuable.
• Experience with government-funded aged care services is advantageous.
• Excellent communication and client-centred approach.

Responsibilities

• Manage caseload of older Australians in home care.
• Conduct client assessments and develop Care Plans.
• Coordinate services and monitor budgets.
• Collaborate with clinical and scheduling teams.
• Advocate for client needs.
