Job summary

APM is seeking a qualified clinician to conduct Residential Aged Care Funding Assessments using the AN-ACC tool. You will work with residents and staff, reviewing records to inform classifications that determine funding levels.

The role offers autonomy, travel across the designated region, and training for professional development, with a flexible schedule between 7:00 am and 7:00 pm. This is a part-time role (0.5 FTE) with fully paid training and support.

Qualifications

• 5+ years’ post registration clinical experience in aged care.
• Tertiary qualification in Nursing, Physiotherapy or Occupational Therapy.
• Current unrestricted AHPRA registration.
• Aged care experience in Australia, including dementia care.
• Experience conducting clinical assessments with standard tools.
• Ability to engage older Australians and residential facility staff.
• Ability to follow direction, work independently and in a team.
• High degree of self-motivation.
• Willingness to locum travel across the designated region.
• Strong planning and organisational skills.
• Excellent communication with consumers, families and managers.
• Own vehicle with comprehensive insurance.

Responsibilities

• Conduct Residential Aged Care Funding Assessments using the AN-ACC tool.
• Review health records and interview residents and staff.
• Triangulate data from desk reviews, observations and conversations to inform classifications.
• Travel across the region to undertake assessments and support other regions as needed.
• Participate in training and professional development.
