Job summary

Advanced Personnel Management is seeking a highly experienced clinician to conduct AN-ACC assessments across aged care facilities in Fitzroy QLD. You will interview residents and staff, review health records, and triangulate data to determine funding classifications.

This role involves locum travel within the region and occasional support to other regions. The position is part-time, three days per week, offering autonomy, paid travel, and access to comprehensive training with CPD points, plus

Qualifications

• 5+ years post-registration clinical experience across aged care
• Tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy
• Current unrestricted General Registration with AHPRA
• Aged care experience including dementia
• Experience conducting clinical assessments using a range of assessment tools
• Ability to engage older Australians and stakeholders in residential facilities
• Ability to follow direction, work independently and as part of a team
• High degree of self-motivation
• Ability to locum travel across the designated region
• Strong planning and organisational skills
• Excellent communication with diverse stakeholders

Responsibilities

• Conduct Residential Aged Care Funding Assessments using the AN-ACC tool across the Fitzroy QLD region
• Interview residents and staff and review health records to inform AN-ACC assessments
• Triangulate data from desktop review, observations and conversations
• Travel to aged care homes and provide regional support as needed
• Attend 4 days of training in Sydney with Department (expenses paid) and update skills
