Job summary

SmartRecruiters, Inc. seeks a Part Time RAC Funding Assessor to join the regional team across Fitzroy, QLD.

The role involves travelling to aged care homes to conduct Residential Aged Care Funding Assessments using the AN-ACC tool, with autonomy and peer support from a RAC Funding Senior Assessor. You will bring 5+ years of post-registration clinical experience in aged care, current AHPRA registration, and strong interviewing and documentation skills.

Qualifications

• 5+ years of post-registration clinical experience in aged care.
• Current unrestricted General Registration with the AHPRA.
• Australian aged care experience, including dementia.
• Experience conducting clinical assessments using a range of assessment tools.
• Ability to engage older Australians and stakeholders in residential facilities.
• Ability to locum travel across the designated region to undertake assessments.

Responsibilities

• Conduct Residential Aged Care Funding Assessments using the AN-ACC tool.
• Travel to aged care homes across the region to perform assessments.
• Review health records and conduct interviews with residents and staff.
• Triangulate data from interviews, documentation and observations for funding classifications.
• Collaborate with RAC Funding Senior Assessor and provide regional support.
